Pastoral Care

Local church ministers frequently visit Lexington Medical Center to provide spiritual care for our patients.

Volunteer members of the Lexington Medical Center's Chaplaincy Program are also available for your spiritual needs. This group represents most of the denominations in our community and is trained in counseling and in providing spiritual assistance.

FaithHealthNC

Lexington is serving as the pilot site for FaithHealthNC, a movement that links local churches with Lexington Medical Center and local human services agencies in a shared mission of health, healing and hope.

FaithHealthNC is funded by Wake Forest Baptist Health and is free to participating congregations. FaithHealthNC strives to improve health by combining the caring strengths of congregations, the clinical expertise of health care providers, and a network of community resources. FaithHealthNC is about helping people navigate a complicated health system to find the best possible health care or resources to meet their needs.

The local program is guided by Rev. Dianne Horton, M.Div, BCC, Manager, Chaplaincy and Clinical Ministries at Lexington Medical Center.

16 local churches serve as FaithHealthNC churches. Participating congregations appoint volunteer Congregational Care Coordinators (CCCs) to facilitate care for members and their neighbors during times of illness. When illness strikes, volunteers spring into action to assist in any way needed. Help could be in the form of assisting during hospitalization as a second set of ears and eyes when consulting with physicians about an appropriate plan of care. Help could also mean assisting in the navigation or resolution of confusing health insurance issues or linking the patient with needed community resources.

Post discharge, Congregational Care Coordinators arrange transportation to and from doctor’s visits, ensure the availability of adequate nutrition and prescription medication, and help create a safe home environment.
